How to Schedule
Testing is available Monday – Friday from 8am – 4:00pm. Call 567.940.9911 to schedule your appointment.
Examinees must provide the following information to reserve an appointment:
- Full Name
- Address
- Phone Number
- Validation Date of Permit
- Permit Number
Testing Requirements
You must have:
- Valid driver’s license
- CDL Permit must be dated 14 days prior to the scheduled test date
- Must be on the FMCSA TPR registry
Payment
Once we have confirmed a testing date and time with you, a link will be sent via text or email to the tester. If text or email is not a viable option for you cash payments are accepted at the Apollo Adult Education office Monday – Thursday from 8am – 6pm.
Payment must be received within 24 hours after the appointment is made. No exceptions. If you do not pay your fees, you will lose that test date and time.
We do not accept American Express.

Test Cancellations / Refunds
OHIO REVISED CODE 4506.09
An applicant for a commercial driver’s license who schedules an appointment to take one or more, but not all, portions of the skills test shall be required to pay an appointment fee equal to the costs of each test scheduled, as prescribed in division (E)(1) of this section, when scheduling such an appointment. If the applicant appears at the time and location specified for the appointment and takes all the portions of the skills test during that appointment that the applicant was scheduled to take, the appointment fee shall serve as the skills test fee. If the applicant schedules an appointment to take one or more, but not all, portions of the skills test and fails to appear at the time and location specified for the appointment, no portion of the appointment fee shall be refunded. If the applicant schedules an appointment to take one or more, but not all, portions of the skills test and appears at the time and location specified for the appointment, but declines or is unable to take all portions of the skills test that the applicant was scheduled to take, no portion of the appointment fee shall be refunded. All scheduled tests must be cancelled or rescheduled at least 48 hours prior to their test date/time, excluding weekend hours, or the payment will be forfeited.
Reschedule / Fees
Examinees may re-schedule their appointment prior to 48 hours of their originally scheduled appointment time without forfeiting their fee. OAC 4506.09
Examinees must call the CDL Testing Office at 567.940.9911 to cancel the appointment. They may leave a voicemail message if during non-business hours.
Late to Appointment
Examinees are expected to arrive prior to their testing time and ready to test. Examinees who arrive late will forfeit their appointment fee. OAC 4506.09
Documentation
All presented permits must be legible, sealed, legal and valid. No person will be permitted to test with problematic or unclear documentation. Must bring license AND permit to be able to test. Vehicles must have a manufacturer’s VIN plate that clearly shows the GVWR. Registrations will not work, paperwork from the manufacturer with GVWR is the only other proof that is permitted. Persons not having proper documentation will have to reschedule and pay another fee.
Vehicle
Vehicle and trailer must be safe and in good working condition.
Vehicles used for testing must have seat belts for front seat occupants. ORC 4513.263
Please do not use loaded trailers for testing.
The examinee must be driven to and from the exam center by a licensed driver. That co-driver must present a valid driver’s license for the class of vehicle the examinee is being tested on. Vehicles can be dropped off at the testing facility 24 hours prior to the test.
